I need a Guest Rider Permission Form, does anybody have something like they'd be willing to share, or point me in the right direction? I'd like to copy and modify it for my own use. Even just the legal wording would work, I can figure out the rest...
I did talk to my insurance company, I am covered they just don't have a standardized form for what ever reason.

I used to work for a company that had forms like that if we wanted to take someone with us on the route,..
"Hold harmless and indemnify" is a phrase I remember from that form,.. -
If you are leased to a company you will need a permission form from them. If you're running under your own authority just make a copy, preferrably with a letterhead, giving you permission to have John Doe (it has to be individual specific) on the truck with you. Give yourself another title ie: operations manager and sign it. I know it sounds like moose muffins bit it will assure you clear sailing if you run into a snarky DOT type.

This is what I have worked up so far. Borrowed, sliced, diced and spliced from other web sites, and put into something usable by me. Please feel free to borrow for your own use. Improvements are encouraged. I still need to call OOIDA and see if they have a cookie cutter form that would replace my work. Didnt see one on their website in the forms section.
PASSENGER RELEASE, INDEMNIFICATION AGREEMENT AND RIDER AUTHORIZATION
The undersigned, being of lawful age, and with knowledge of the hazards involved in the transportation industry, hereby voluntarily agrees and/or represents as applicable:
1. That in exchange for free transportation on a company owned vehicle or driver leased vehicle, the undersigned (hereinafter Passenger) hereby releases and forever discharges ___company name___, its affiliates and subsidiaries, officers and employees from any and all claims, losses, injuries or damages, including personal injury or death, resulting directly or indirectly from the Passengers presence as a passenger on a ___company name____ truck.
2. Passenger agrees to indemnify, defend and hold ____company name____ harmless from any injury or loss resulting to _______company name______ or any third party arising from the Passengers presence on any Company owned or Driver leased vehicle.

If your an independent O/O and not leased to anyone..is this still needed?
My insurance company told me anyone I bring in the rig with me is automatically covered w/o any need to tell them.
I did draft a 'passenger policy' as part of my 'business' needs but all it states is:

I dont see why you would need something in writeing if running under your own authority and insurance if your the one driving, get pulled over cop asks just say yes i give permission dont see the need for a piece of paper and i've never had an issue with a passenger in the truck with me.

Dude, I hear ya but I was TOLD I had to have one. As stupid as that sounds...

Somewhere in the FMCSA regulations it states that a driver needs to have permission in writing from his company to have a passenger that is not an employee of the company,...
Therefore if your a one truck o/o running under your own authority you need permission in writing form yourself to have a passenger ridding with you,.. -
Only took a few minutes to draft anyways....
But ya, this reeks of utter stupidity...
"I give myself permission to have passenger X riding with me..."
