Is annual DOT insp. done while at one carrier good when I move to another carrier?
I will furnish the paperwork to the new carrier to have in the office.
My guess is yes.

yes..............but when you sign up with a new carrier they may require a fresh inspection

Sticker is good for a year - as long as it was done by a "qualified inspector". FMSCA is kinda specific as to what THEY consider "qualified", but most carriers in-house shops meet that requirement (which is not to say that their mechanics are worth a crap - but just that they meet the qualifications to be able to issue the sticker).
I'd find a CVSA Level I pass sticker of more value personally - but, the annual is a DOT/FMCSA requirement.

On the sticker it must say "address where maintenance records for this truck are kept" or something to that effect. You also must have DOT number on the sticker, it will obviously be different.
The answer is NO.
You must get a new inspection.

Nah, the old sticker stays with the truck, DOT doesn't require you to change it when you change companies. The rule says it has to be done annually and that's it. If it's really bothering you, just buy those annual inspection reports and sticker from a truckstop or local truck parts store. fill them out with the "right" information. Just don't put your name on it. Some might think this is falsifying, but it really isn't. The truck is in good condition like you say

if it's a requirement. how come i've only seen 3 stickers in 14 months.
i've only seen about 50 cvsa stickers.
