I always thought of it as more of a demeaning term. Your nothing more than a hand to steer the truck down the road. Either way I find it very annoying.

When I was younger (20 about) my first wife's dad was a driver, then he owned a truck tire business. This was about 1980. He used the word hand quite often, but not always. Reflecting back now, I realize he used it as a term of respect. I remember two situations where drivers looking for tires were rude and what I thought was somewhat degrading to him. He called them "bud"!
