My orientation papers say that if I've been under a doctor's care in the last 5 years, I need their release.....so my question is, do I need releases from the following Dr.'s?
3 years ago I was treated for anxiety caused by stress when my mother died (very short term...lasted about a week).....Last January I had surgery....Last September I was treated for stress fractures in my feet.
Dr.'s Release??

I think what they mean is that if you have been under constant care of a doctor for a mental of physical disablity you need to get a release form signed by those you were under care of. It doesnt sound like you need it, but it doesn hurt to ask.

I was afraid of that. I can easily get the last two, the first one will have to come from my Dr. that lives 3 hours away....I just hope he's still in business.

To CYA ask the company exactly what do they mean. It's better to list everything they want then to have them come back later and say you're trying to cover something up. There's nothing what you listed that would prevent you from driving. Sorry to hear about your mother.

Thank you, Gashauler. There was so much stress caused by my dad and brother that I was having anxiety attacks and my BP went through the roof for a few days.
I talked to one of the recruiters and he said to go ahead and get the releases, even tho I probably won't need them. So I will have 4 releases to take with me....one being the only one from my current Dr. who is treating me for acid reflux/gerd and high blood pressure brought on by some stress earlier this year. Without my BP pills it runs 150's/90's....with it I stay in the 120's/80's. -
instead of driving the three hours have that doctor fax it to the company and send one by snail mail to you.
Jimbo60 and simplyred1962 Thank this.
