HOS has changed ... and the DOT drives around more in cars and pulls you over at random more than they used to ... they are trying to force the electronic log and govern all trucks ... I guess if I was in your shoes and had 20 years in and was coming back for something to fill my time, yeah I would come back and do it for a while, if the regulation/BS gets to much for your liking you can always hang it up again ...

FMCSA has mandated that all drivers have to now wear dresses while on duty. So if you plan to start driving again you'll need to go shopping with your wife to pick out some nice ones. The DoT prefers you wear ballroom gowns.

Most companies will require a refresher course, but not all. It's not that they doubt your driving abilities, it's that so much has changed with hours-of-service, electronic logs, etc. I recommend you pick a company with their own refresher course, then in a few months, move on if you wish. There are dozens of companies with refresher courses.
Here's a few off the top of my head:

    Celadon
    Paschall Truck Lines
    Western Express
    Pride Transport
