What is usually the life of these transmissions when used above average? Mine currently had 650k miles until it gave up? What is the proper way to do preventive maintenance on the transmissions?

If the transmission is properly operated you can get 2 million. It all depends on how it is used or abused.
I think Eaton usually recommends about 500,000 miles with synthetic lube. As far as preventative maintenance, all you do is simply check the oil level/quality every service. Some trucks have tranny coolers, so with those you would just inspect the cooler and lines for leaks and condition.

Verdict: we removed transmission and found more problems. Transmission cooler and PTO is filled with metal shavings from the transmission grinding its gears. The problem was the transmission did not have much oil in it and that caused it to give out. In addition, my driver drove the truck on 9th gear and end up destroying the clutch. I have to buy new clutch as well and resurface the flywheel. All this cost an arm and a leg.

That sucks, sorry to hear all that. I would have been replacing the clutch anyway since the trans was out.

Yeah, its pretty horrible. I kinda wish that the driver have some type of a 6th sense to feel these problems and get it to my attention before it will cost thousands of dollars.
