How many miles on it? any engine rebuild yet? Some ISX Cummins like to crap out around 500,000 miles. See if you can get detailed service records for it.
Normally I think the DPF needs to be changed and system cleaned at a bare minimun of every 200,000 miles, more often depending on idling/city driving. Quite an expensive ordeal. -
Yea DPF scares me iv heard some real horror stories truck got 350xxx miles on it .... I'm gonna be running a lot local stuff not many miles at all hopefully I get lucky and don't have any problems

I wouldn't recommend it, but if you must buy it offer 28ish and see how desperate they are to sell.
That should tell you something ....

The actual truck itself is ok. The engine is also pretty good other than egr coolers are a 100% failure rate at around 600-800K or so. If the egr cooler miraculously has never been done, it will be due any time. The real issue is the after treatment. DPFs and DOCs do need periodic cleaning. The DOC also will not last forever. The aftertreatment injector also needs to be replaced every so often. Before buying any truck with DPF/DOC aftertreatment you need to see some kind of maintenance records as to when they were cleaned last. If the truck has a ton of hours on it the DOC is approaching the end of its life. If it still has the original DOC and/or they cannot produce cleaning records, I would pass.
