Is it popping out the exhaust?

yes it is at the exhaust. not sure if it is a piston ring or camshaft or governor problem.
or maybe something else, any ideas ? -
Sounds like a problem with an exhaust valve. Could be that the adjustment on one valve is too tight (no lash). Does it run better when the engine is cold? Does this engine have Jake or exhaust brake?

have checked valve adjustment, inlet is set at 0.016 and exhaust is set at 0.024
the engine is a 1987 e6 285hp with dynatard with ambac pump.

If you increase the revs by say 200-400 rpm, does it even out and sound sweet or does it stay the same?

A piston with excessive carbon deposits on top or a dying, dribbling injector will give you similar symptoms. Your fault is "sometimes" and "sometimes" faults could be a number of things. When last did you run fuel system cleaner through it? Doing city runs and not getting a good pull up hill or a motorway run will give you grief in the end.
