on the side should be a metal plate with the model number. or go to Pete with your trk. s/n number and they can look up the model 5th wheel you have

Ordering component parts by truck serial number can be a problem if you do not know they are original. Holland 5th wheels are very good but frequently require welding and grinding to fit the yoke to bottom of the plate. They used to come with instructions to do that but the last kit I saw just had allowable wear limits but I welded and ground to fit. Just trying to let you know it is probably not as easy as you may thank, consider exchange, new, or professional help. If you have those abilities and desire to it's not that hard but the liability is high. Anybody please correct me if you disagree.

there should be a plate somewhere on the front of the fifth wheel or the side indicating the model and serial number. It's very easy to rebuild. I've done it myself takes about an hour and the rebuild kit costs around $280. Only thing I would suggest if they have someone hold the diagram for you to put your hands get so greasy. I would however make sure the top plate of the fifth wheel isn't worn out as well. because a new fifth wheel is only about $800 depending on what brand you purchase.
