First question can't be answered with certainty. Time is dependent on when your trainer feels you're ready and the loads you get.

Not sure bout reefer. Flatbed starts at 36cpm.

I thought I remember Primes training being very lengthy. 4-6 months with a trainer. Here is the link with the info:

That needs to be updated. That's old. Doesn't take that long.
