I need to go get a new dot physical soon to comply with the new regulations, and then I have to send the NC dmv a certification by January 30, 2014.
I plan on getting new physical card next week, as mine is expired.
Should I just got the physical and skip the drug/alcohol screen as I will be taking a driving job in Jan/Feb and I know my new company will do the drug alcohol testing at the time of hire? Or is the drug/alcohol screen included with a dot physical?

My card expired in March since i was on med leave not driving all i had to do is sign a certificate saying i don't drive from state to state and whenever i get new card i have to take them a copy until then no worries

If you already have your CDL I wouldn't procrastinate and get 'er done! If you don't have a CDL and plan on going to a mega carrier you can get your physical at their expense.
Physicals and drug tests are two separate programs. -
You should do something pronto. The intent of the "self-certification" thing is to keep drivers "current", and a lapsed DOT physical could lead to the state suspending your CDL status forcing you to go jump through all the hoops again and enduring all the hassles involved in obtaining a CDL. The drug screen is not needed and is not part of the standard DOT physical. Those are only used in pre-hire physicals to meet employer demands.
