Dac report

    Drivers by law if a company pulls your DAC report they are suppose to send you a copy of it to see what they are seeing. I have had a few sent to me but not all companys do this.

    I have a copy of my DAC report and there Is one Incident on my report from 5 yrs ago but thats all It says.
    To get additional Information I have to Fax a DAC Drug /Alcohol Release form to DAC. Can I get this Information? Were can I get a
    DAC Drug /Alcohol Release form?

    It is similar to a "Credit Report" but it contains all the MVR information as well as Internal Company reports (of those companies who subscribe to the service). There is NO review of the legitimacy of the information in a DAC report, so there is a good chance that information on your DAC is erroronious or outright false. It is up to the driver to contest any wrong or misleading information, and USIS (United States Information Services) doesn't always remove this false/misleading information. You should pull your DAC at least once per year.

    Hello Y'all! :wave:

    I just mailed out the request for my husband's DAC report, below are the things you will need to mail and the address to mail them to:

    • Provide your full name
    • Mailing Address
    • Social Security Number
    • The name of the company that turned you down for employment (if applies)
    • Photocopy of your Driver's License and Social Security Card for I.D.
    Mail to:

    USIS Commercial Services
    Consumer Consulting Department
    P.O. Box 33181
    Tulsa, OK 74153

    Or you can call them at (800) 381-0645, and they will tell you everything I just told you :biggrin_25523:!

    You are entitled to one free DAC report a year and/or turned down for employment due to DAC report. If neither applies to you, don't fret, just pay the $10.00 fee and you can receive one too.

    USIS (formerly DAC) Has quite a racket going. They pay companies 3 bucks for your info and turn around and sale it to you for 10 bucks.:biggrin_2554: I say we figure out a way to charge them a fee for any bogus info.

    Not to mention, they charge "participating" trucking companies $1000 per year.

    I know when I was first looking for a job I applied to swift and they sent me a copy of my DAC.

    I recently got my Class A permit and my physical card. I am currently on a layoff and waiting for unemployment to call me in for orientation so I can sign up for my CDL class. Who would be the best to sign up with for training?

    Hmmm I've never gotten one before.. then again I haven't been denied employment. Although I should check anyway. Thanks for the info!
