It look like you have a mechanical problem, the engine brake stay On eventhough you depress the accelerator it's the reason why your heard misfiring, the ecm start to inject fuel but some exhaust valves stay open, there more than one possibility, faulty engine brake solonoid, slave piston start to seize in the exhaust rocker.
I recommand you to don't use anymore your engine brake up you check it because i saw severe engine damage in the past due of seize slave piston.

Isx is famous for turbo actuators .. You may need to do a rawze style clean up to remove soot

    Thank you guys!
Haven't fix the problem yet. It still persists. Also I've got another problem. The thing is batteries don't get charged enough. Voltage jumps between 11.7 to 12.2 V. New alternator and new batteries.
    Alternator produces 13.7 V.
Does engine brake solenoid get shorted or something then those 2 symtoms are related or not?

Ok. Finally I've got few answers.
Engine break harness was freyed, dry rotten. Replaced it and problem with engine break solved.
    About the electric problem.
Cleaned and tightened battery, starter and ground connections.
Added ground cable between engine and frame, between cab and frame.
Now it charges 13.4 wich is I still have a load or short somewhere.
    Thanks guys for your help.
