You can get it in any state. They write your license address on the paperwork.

In ky, the driver is responsible for making sure his medical is certified. If you depend in others to do for you, you take chances I'm not willing to take.
Sidenote, does your company pay for it? If so, go anywhere! -
My company doesn't pay. I'll definitely be taking it to the dmv myself. I don't trust anyone to care about me more then me. Now to find a place to get it done as cheap as possible

Think the cheapest I've ever seen or sent someone to was a c-note. Do yourself a huge and make sure the dr you see is on the national registry tho bro. If you're shopping cheap you may run into guys that used to be able to give dot physicals but since the new laws they're outta the loop.
Good luck man. -
Ya I saw the official list. Will definitely be checking it.

I live in Texas and got mine in New Mexico. No particular reason other than it expired while I was there. I think I paid $65

Find a Concentra. Around a hundred bucks and you can be confident that the Dr.ison the registry rather than taking a roll of the dice with a chiro somewhere in upstate VA or some other crackpot.

The fmcsa posts a list of all the certified drs. So I'll be cross referencing against that
