Sample shot of filled out IFTA form available?

Hi,

I am doing the IFTA taxes for the first quarter '08. I am based in FL, but the truck has run in 4 different states these past few months. I had printed out mileage sheets for all the pert info to be recorded, so all the figures are there on paper (boy, I figured it all on paper, but from now on I'm going to do all in a spreadsheet when the weekly sheet comes in!).

*Anyway, I have gone over many of the threads in here about the IFTA taxes and although it's been explained several times, I am still confused about a few things I need to enter on the form. It's my first time doing this, and it's not that easy for me to understand without actually SEEING a sample of it.

Does anyone have an actual sample of the form filled out that they wouldn't mind sharing? Unless someone would be willing to me PM me with a little help (if I show them what I've done so far), I would really appreciate it.
