This happens more often than you think.
Inexperience driver do this everyday, fortunately it is in an area were there is little or not traffic.
Analysis
Because drivers are not well trained in all things that could happen they tend to feel the adrenalin that they get from driving such a large powerful unit. As a new driver experiences more time on the road he become Self Educated(OJT), On The Job Training. This can be a very expensive way to learn or if the driver that has been taught correctly and uses his tools of the trade he will not make these mistakes.
A driver looses a trailer to due to about three things:
1. He did not look at the locking pins in the fifth wheel to see it was locked.
2. He did not complete his Safety Walk Around Inspection before starting out.
3. He did not Inspect or check is unit out after he was parked at a truck stop or rest area and someone pulled the fifth wheel pin release. This happens when driver cut off other motorist. Some people are crazy.
