OSHA fines trucking firm $9,100 in crushing death

OMAHA, Neb. — The federal Occupational Safety and Health Administration has fined a trucking company $9,100 for failing to take precautions that could have prevented a fatal accident.

OSHA’s Charles Adkins said Wednesday the accident that killed 62-year-old Brian Cellar near Endicott was preventable. OSHA says Omaha-based Wynne Transport Service Inc. failed to report the accident to OSHA within eight hours, as required.
