Tug test - FAIL
Poor guy had fun cranking that up.

I would guess a tug test was never initiated.... To far away from the dock & usually when your just hooking up.... A tug test is done before you raise the dollies

Yeah, no tug test, though he wishes he had. Seemed like an appropriate heading though

Remember that a "tug test" doesn't always work, especially on an older truck where the 5th wheel jaws are worn or sometimes a trailer with a worn pin. Take the extra seconds to look and be sure the jaws/lock are closed on the pin.
Best idea on any trailer you didn't personally drop is to look first before you back under. Yard mules with hydraulic 5th wheels generally sit a few inches higher than your 5th wheel height. So do trucks with 24.5 tires (fleet trucks are generally 22.5). Embarssing as heck to ram the front of the trailer into the back of your cab cause you underrode the pin.
