Your Medical Card and Long Form are the property of whom ever the physician issued it to! It is
against state and federal law to with hold a persons medical record! Not only that you are suppose to
have the long form with you because they request to see your long form! Also, If you have been
said to have sleep apnea and are on a C-PAP machine, you are too have a copy of your Longform on
you along with you sleep apnea doctors name and phone and a copy of your 90 day download! -
I still have mine from god knows how many decades ago. -
Future - Medical Officer will turn the information directly to the state and the driver does not have to do anything. -
Mark is spot on, it's all in the databases now. When the lawman runs your D/L, it'll all come up, and then some.....

Ummmm..this thread is 7 years old. And the rules have changed since. It's probably meaningless by now.

Regardless who paid for it you signed for it at the doctor's office in your name go back and get a copy it's your health record and by law it attaches to your drivers license.
