Medical Long Form

    One reason people go to college is so they can learn to argue and debate at a professional level without fighting and name calling. I don't need a college degree to figure out that's better way to prove your point. Intelligence is more powerful than having a set. Fortunately I have both! :biggrin_2559:

    Like ursus says, what's the big deal? Just put the piece of paper in your logbook cover.

    But I do know when I worked for Westside Transport, they paid for the physical, the doctor sealed the long form in an envelope and we had to return it to the company with no copies allowed. I was not concerned as I had a 2nd personal medical long form in my possession. They didn't want drivers quitting and having a free physical in their possession. So there is something they know.

    I'll do a little research and self educate on this Hippo thing. brb :)

    Interesting tit for tat guys...... In Texas the long form is retained by the carrier / doctor because:

    1) They paid for it

    2) No one will hire you without it. ( They need the form in their file and keeps u from jumping )

    Also, I have never been asked by any LEO anywhere for my long form. Ha! I don't have it...the carrier does..... so the card has always done the trick.

    This something new?

    I don't know if it is new or it could just vary by State.
    Indiana requires one when you get your CDL and then a new one every two years. The State wants their copy. They currently have a mailing address and fax number for this.
    I have read that they are going to set up a system where an approved doctor can send the form in directly from the office on-line.

    Now that I learned, my interpretation is you must have a long form in your possession since it is a supporting document. Employers paying for physicals must let you have the original long form. They can have a copy.

    HIPAA covers protection of health care info and health insurance claims. Physicals are a determination if one is fit for duty and is not health care. They don't find something disqualifying and treat you for it. There is a list of people that must comply with HIPAA and all are health care institutions. A trucking company in possession of your long form is not covered under HIPAA. A driver in possession of the long form is not covered under HIPAA. The driver is protected from health institutions releasing health care info is all. Most of us sign release forms too when we sign on with a company. DOT has power over HIPAA.

    An interesting find in researching, I found federal and state drivers are not required to have a DOT physical exam.

    Better watch out.

    You will be called billy big rigger here.

    It is what I was stating when they asked for the long form on the inspections. It is something they have been doing for a couple of years. I have always carried mine since I pay for it. I am leased to a company.

    In addition, if they find prescriptions in the truck, they are also on the medical form. This includes glasses.

    The MEDICAL EXAMINATION CERTIFICATE is not the long form. It the wallet card, the long form is called the Medical Examination Report

    This is a moot point. You may be put on meds 6 weeks afterwards and they would not be listed. In 28 years of trucking I have never been asked for my long form also I have never had a officer in my truck.

    All states should now ask for your MEDICAL EXAMINATION CERTIFICATE before you renew you CDL, some may have to have the long form. Kentucky only asks to see a current MEDICAL EXAMINATION CERTIFICATE (wallet card) to renew the CDL.

    You are lucky then.

    Mine has been requested a couple of times. I gave them the wallet card and they came back and requested the long form.
