Are there states that require a driver to produce a medical long form?

if you mean a company...then yes, they are required to have the drivers long form on file..they can accept a current physical as long as it is not expired, or they can choose to give the driver another one before they hire himor her ,if you mean the d.o.t. then legally they can not ask for your long form...all they are entitled to is the medical card showing that the driver is indeed med. qualified. the other info on a long form is considered personal info. that they do not have the right to access.

some companies you hire on with, if they give u a new physical, they wont even give you a copy of the long form..they keep it on file, just a copy of the medical card.

§391.41 Physical qualifications for drivers.
(a)(1)(i) A person subject to this part must not operate a commercial motor vehicle unless he or she is medically certified as physically qualified to do so, and, except as provided in paragraph (a)(2) of this section, when on-duty has on his or her person the original, or a copy, of a current medical examiner’s certificate that he or she is physically qualified to drive a commercial motor vehicle....

that is taken from the federal motor carriers safety reg handbook......your certificate..is your med card.....

Those are the federal rules.

Each state operates their own rules.

Indiana requires you to have one just to get your CDL Class A.

Most companies I been with send you for a DOT physical and pay for it. The long form is put in a sealed envelope to give back to safety while the medical certificate was given to me which I keep in my wallet. Companies don't want you having possession of the long form they pay for. Say you quit, they don't want you having a free physical for the next company.

The ones I paid for I put the long form inside my logbook in a pocket.

I never had more than a certificate check in any inspection or stop. It is sufficient proof that you are medically qualified. I don't see why a state would require the long form as the info is redundant if a driver is qualified with a certificate in their possession.
The only time I could see a long form used is for a wreck in legal proceedings.

I've been asked for and shown my long form to a few DOT officers. The card just shows that you have completed the physical and is a representative showing of the long form. Why an Officer would want to see the long form is beyond me unless he wants to be sure that the card is legal. The info on the long form isn't all that "personal" unless you're scared they might see your height, weight, BP and vision which just looking at you the officer can tell that you're 5' something, fat or skinny as a rail have a set of bottle caps on your nose and looking at the smoke coming out of your ears have a case of high blood pressure so whats the big deal about the little bit of info on the long form?

Heck you can download the stupid thing and falsify all the info on it along with a card so the officer really has no way to tell if it's real or not by comparing the two! If the information is so personal, why do we all sit around the dinner counter and compare BP's? There's really nothing that personal on it! Get over that big brother crapola! It won't br freaking long before it's put on our drivers licence or even on that silly CSA sheet!

The reason a lot of companies don't want you to have a copy of it is because they paid for it and don't want you to have it for incase you don't make it through orientation or you quit!

Now if you paid for the medical keep your copy and give the company a copy but do not give then the original unless they are making the copy! This is why when you go in and do your DOT they will either hand you a sealed envelope or just the pages.

Me: I carry 2 flash drives with my medical information on it and on the side of the drives I have in gold metallic ink "ICE" for "In Case of Emergency"for if I'm found face down in a parking lot. I WANT the medics and hospital and police to know who I am and here's my history to see if there is something they need to know that I just might not be able to answer due to my condition. I also have a note on my cell titled ICE with basic information like blood type normal BP meds and general info AND my hospital information where all my medical information is kept and a note telling them to access the flash drive on either key chain for a downloadable signed permission form that my hospital where the records are kept know about, to access my medical info AND my living will telling them what to do in case of the worst!

This is something we should all do! It's easier for the triage nurse to read this information than the staff having to guess at what might be going on with you!
