    Um,,, you might want to KEEP your long form or copy of in your truck at all times, or some smart-arsed DOT can and maybe will give you citation for not having it.

    Not a driver yet. Graduated from school in June, got CDL, and did not apply for jobs as I had to finish up business with my old job. I am going to start applying for jobs this week, hence the concern with losing the med card. And I don't have a copy of my long form. When I got my CDL, the DMV asked me for it and I didn't get it back. Were they supposed to give it back to me?

    I got the physical done at Concerta.

    it's already tied into the system.

    i got pulled over in indiana. faxed my new license and card to the company and forgot to put back in my wallet. so i had neither iwth me. but the cop looked it up. knew that i was legal. and sent me on my way.

    i don't think we have to carry the cards anymore. and probably not our license anymore if we know the number. but since we all need identification some times. i'll keep my license with me.

    and noooooooooooooooooo we don't need the long form. i don't know where some of you get that idea. the long form stays home.

    i don't even know where my long form is. 7 medicals and they've all gotten lost.

    Final rule.Summary:

    The FMCSA amends its regulations to keep in effect until January 30, 2014, the requirement that interstate drivers subject to the commercial driver's license (CDL) regulations and the Federal physical qualification requirements must retain paper copies of their medical examiner's certificate. Interstate motor carriers are also required to retain copies of their drivers' medical certificates in their driver qualification files. This action is being taken to ensure the medical qualification of CDL holders until all States are able to post the medical self-certification and medical examiner's certificate data on the Commercial Driver's License Information System (CDLIS) driver record. This rule does not, however, extend the compliance dates for States to collect and to post to the CDLIS driver record data from a CDL holder's medical self-certification and medical examiner's certificate.

    I had my medical card attached to my license by the state back in February. So yes, I don't HAVE to carry my short form medical card anymore.

    You never have needed to have your long form with you.

    Starting on January 30, 2012


    When you:

    • Apply for a CDL (A, B or C class)
    • Renew a CDL
    • Apply for a higher class of CDL
    • Apply for a new endorsement on a CDL


    Your application must include a CDL Medical Self-Certification Form (PS33203), on which you will need to certify that you operate in one of the following categories:
    Category 1 – Non-exempt Interstate, subject to 49 CFR part 391 (must also provide a current medical examiner’s certificate and any waivers indicated on the certificate)

    Category 2 – Exempt Interstate, exempt from medical examination requirements of 49 CFR part 391


    Category 3 – Non-exempt Intrastate, subject to state medical examination requirements of Minnesota Statutes, chapter 221 (must provide current medical examiner’s certificate and any waivers indicated on the certificate)


    Category 4 – Exempt Intrastate, exempt from state medical examination requirements listed in Minnesota Statutes, chapter 221.

    Refer to the Self-Certification Just the Facts for guidance on selecting a Category.

    Minnesota Statutes and Federal Motor Carrier Safety Regulations require the self-certification. Reference: Minnesota Statutes, sections 171.162, 221.031, and 221.0314, and 49 CFR 383.71 and 383.73

    You are correct, you never needed to carry the long form. That is for the file in the office.

    However, you still need to carry the short form. Just because you filed a self certification with MN does not mean other states have access to that information at this time.
