The correct procedure is to go out and crash your truck in order to bring yourself into compliance.
seriously though, I don't know how to go about getting it off, but I'd probably start here:
http://www.fmcsa.dot.gov/mission/web-policies-and-resources48stater, GearWarrant, tucker and 3 others Thank this. -
If you are a member of OOIDA, call their compliance department. They have helped others to get their CSA cleared up.

That's a lot to happen your first year. Have you had any other violations that maybe data got crossed by maybe someone going to lunch in the middle of entering your info? This is a wild story.
Meltom Thanks this. -
slowpoke89 and CondoCruiser Thank this.

It can happen, but it is unusual. If it's not your crash, though. Use DataQ's and get it off. All the larger companies and a lot of medium and even some smaller ones run your PSP pre-hire now. -
Believe me....it can happen. I received a letter once stating that my license would be revoked on a certain date for non-payment of fines. It just so happened that the county where the violation occurred was a county I had never been to. I called the department that issued the letter and they denied any wrong doing...of course. So, then I called my legal plan. Once a lawyer called them and had the same discussion, they magicly found the problem and called me with an apology. Supposedly, somebody keyed in the wrong info somewhere. Once that had happened, my info was all they saw from that point forward.
This was long before CSA scores and dataq.... mistakes can happen. Just dont think that they will want to believe you. I'm sure there are many people every day trying to get legitimate violations removed by claiming errors. Follow the advise already offered... Dataq and OOIDA.... or other legal representation. -
Nothing hard at all to believe about it. Ever look at the enthusiasm level of some of these people who work in these departments?

How did this turn out for the OP?
Had several violations on trucks on my record that were not mine, never mine and absolutely never gotten them off my record yet after one year. Went to to Data Q and did all the necessary work there. Got an email from DOT that said they would be taking them off but they have never come off yet.
The mistake was made by the DOT department of the Houston Police. Yes they have there own DOT Dept amazingly enough.
