Company Name 395.8(d) - Driver logs

    FMCSA says to list Carrier name, and main office address.

    1. Carrier name is XYZ LLC. Can a driver just write XYZ, or does it need to say LLC?

    2. Headquarters of the LLC is in Florida. The truck is housed, reports out of, and only works from the office in Co. Should the main office address be Co or Fl? Truck has "XYZ LLC" listed on the truck along with DOT #...

    1. You need the full name.

    2. You need to put the main office address (I.e. the address of record on file with the DOT) regardless of the fact that the truck doesn't run in that area.

    Ours have the mailing address, including street address, PO box number, city, state, and at least the 5 digit postal code for the company address.

    Home terminal address is on the next line.

    I run strictly North Carolina to Virginia and back. My home terminal is Lakeland Florida, and that is what is on my logs

    since logs are based and done on the time zone of main location. i'm guessing that's also the address, regardless of where truck actually runs.

    i'd hate to be you guys running on the opposite side of the country. or living.

    When I was with Swift everyone's logs had the address of their corporate HQ in Phoenix but we were required to run our hours based on where our assigned home terminal was. Living in SD it was the same as home half the year due to AZ not having DST which got confusing.

    I'm with Crete now... corporate's address in NE is on our logs and all of us (regardless of where we live) run on Lincoln time. So I guess log time zones are up to your company to decide.
