FMCSA says to list Carrier name, and main office address.
1. Carrier name is XYZ LLC. Can a driver just write XYZ, or does it need to say LLC?
2. Headquarters of the LLC is in Florida. The truck is housed, reports out of, and only works from the office in Co. Should the main office address be Co or Fl? Truck has "XYZ LLC" listed on the truck along with DOT #...

Ours have the mailing address, including street address, PO box number, city, state, and at least the 5 digit postal code for the company address.

since logs are based and done on the time zone of main location. i'm guessing that's also the address, regardless of where truck actually runs.
i'd hate to be you guys running on the opposite side of the country. or living. -
I'm with Crete now... corporate's address in NE is on our logs and all of us (regardless of where we live) run on Lincoln time. So I guess log time zones are up to your company to decide.
