In my view insurance rightly denied this because there weren't enough criteria. Basing this off of neck size and BMI ONLY means an awful lot of skinny people who have it are slipping through the cracks.
From experience. I had a whole range of symptoms. Tired during the day, microsleeps while driving POV (before I had CDL) , loud and excessive snoring, elongated uvula due to the snoring, waking up with headaches, sore all over all the time due to jerking myself awake to breathe while "sleeping" , borderline high blood pressure. Conversation with my doctor asking all these kinds of questions, answering in the affrimative got me the referral for study.
Now if I read the OP correctly, it was based off neck size and BMI, which granted, while it CAN be an indicator doesnt mean squat in and off themselves. If the OP was telling the truth related to the other questions , then he was biased in "Truckers lie", "never came back negative" Which I call BS on.
ME requires sleep study but refuses to order it?

If your GP doesn't think you need a test, then tell the DOT doc to go take a hike. Listen to your GP, they have your history whereas the DOT guy is just guessing. -
Prime, Inc pulled that stunt on drivers....Told Drivers they needed to do a Sleep Study for 2400 bucks and then wear a mask to breathe right ...If there neck size and BMI was above average.
I also read there is a place on the DOT medical examiners report that requires a doctor to at least check if the patient has trouble snoring....
Snoring ? Really....Do all people who snore have Sleep Apnea???? I don't Think so. But usually it is related to a sleep disorder
Maybe the Op should call around and park her slippers under another bunk. -

If it isnt needed then pcp should have no problem stating so in writing.

It would not matter. The DOT doc requires it and would not give me a card unless I have the test done.
In principle, I agree with the letter idea. That is how it should work!
Or better yet, only your PCP should be able to give you a DOT Physical Card. -
Also , what MAY garner some action is this area is contacting your Rep and Senator on this. Perhaps they can get something done in regards to FMCSA creating new logical guidance on this as not yet required suggestion. -

I am not a "sue 'em" kind of person. I am a "get it done" and move on type. I do not believe this should be happening to drivers though. We all need to be speaking up and be heard. Perhaps a letter to my congressman will at least be another voice he can ignore because he is too busy looking for corporate donors! YAY!

The Self Certification of Drivers to the Office of Motor Vehicles states that only a Certified Medical Professional (not an MD) must be registered to administer the D.O.T. medical examiners report. It also states that failure to send a copy to the state office of motor vehicles (DMV) and Licenses will result in suspension of driving privileges. note; Not all Primary Care Providers PCP are on the federal D.O.T. List. So the regular Family Doctor may not be on the list will and may no longer perform the Federally mandated FMCSA Medical Certification.

A Medical card is primarily for OTR Drivers, Some states allow a intrastate only exemption on the CDL...(for local drivers)
Go to another DOT certified Dr (the guy in the white lab coat who will take the cash and look the other way) and have a new test done and submit a New ID Card Medical Card to the DMV.
Or, take a loan out and go do the Sleep Study and determine if you have been correctly diagnosed by the Dr.
And, You can start working a new plan to get you where you need to be without having to do either of the above.
This is not rocket science folks, Sleep Disorders are a reality, as a professional it would be in your best interest to take care it before it takes of you, by literally destroying a good career because of your health issues.
I had the question of snoring raised by a Dr. when my neck was 19", he told me to lose weight! Now my neck size is 17 and there are no more questions about it on my yearly examination.
