OK here we go, not really looking for legal advise, but more along the lines of medical opinion(DOTPhysical are you out there...) 18 months ago the doctor found I had blockage in 2 of my carotid arteries left 100% nothing can be done, right 80% repaired, snuck that one through with a beard. 6 months later a bleeding ulcer/artery ruptured. they cauterized the artery and put 3 clips on the ulcer, and a couple of transfusions later all is good, Dr. said to wait 4 weeks before returning to work, so being the productive person I am I think this is a good time to update all my cardiovascular test/checkups. Stress test was good, echo was also good, only problem was hard to catch my breath after all was done, and was told it was probably from the recent transfusions and the resulting mild anemia. 10 days later getting ready to go back to work but still getting winded pretty easy and still taking forever to recover. Back to cardiologist and he decides to do a angiogram and finds 4 blockages that did not show up during my stress/echo tests, 100, 85, 85, and 75% respectively. Now after a quadruple bypass I am left with 2 aneurisms that can't be fixed, and the 100% blocked carotid (not to mention a severely torn rotator cuff that magically appeared when I woke up). I have been told by every doctor I have been treated by in the last 18 months to not get back in a truck, including the physical therapist working with my shoulder, and I'm not looking to, but I just received my letter from the state that I have to register my physical by may 19th or my CDL will be cancelled, I understand that. I am currently jumping through the social security hoops and have been advised by my attorney to not take the physical, his reasoning is, IF I pass it will blow a big hole in my claim that I can't work. I am no doctor but if I am understanding the regs I am disqualified on a couple different levels right? I'm thinking a failed physical would only strengthen my case, but like I said I'm not looking for legal advise just medical, so my question is would you sign off on me? I know you can't guarantee anything just looking for your opinions.

If concern is losing cdl You can change to intra state class and keep cdl then change back to inter when you feel you can pass. Good luck.

Go to DMV to fill out Med. card form to keep from being down graded.

No I am not a doctor but I was a paramedic and I trained at a medical school.
First, I doubt you'll pass. The way you explain your shortness of breath and your pertinent overall history and recent procedures I can't imagine any MD signing off on your ability to drive.
I would ensure that the physical was preformed by an actual medical doctor and not a nurse or Chiropractor.

If I understand your symptoms, you have one carotid artery 100% blocked and it cannot be opened. The other carotid was opened. When my wife died with those symptoms, the surgeon explained that a small amount of blood flows across the brain from the opposite carotid artery. Her good carotid was only 70% blocked, but not enough blood flow to keep the brain from slowly dying on the opposite side.

I think it would be very easy for you to fail the physical. As soon as you mention shortness of breath and dizzy upon standing you have failed. They don't like to hear the word dizzy at all.
It is probably devastating to have your CDL downgraded even though you wont be driving a CMV any longer. I would look into changing the status of your license,just so you can still have the cdl without taking a physical.
