It's the drivers responsibility to make sure your medical certification is registered with you state licensing agency. States have different policies if/when you let the certifications lapse. On the door of the DMV here it says they will not accept long form, as it is medical history which is basically none of their business, they just check the small card and put it into the computer, of course the doctor/registered nurse has to have a registry number and have put it into the computer also.

Never assume the school or examiner sent them. It your DMV doesn't get them and the card expires, they will downgrade your license to a regular car license until you provide them with an updated medical card. If you get caught driving a cmv with a downgrade, that's big trouble.

Yeah as I think back I got my physical then went to the DMV to take my test for my permit, which I showed them my medical card ( not longform ), thus is how it was put on record. I didn't notify them of the newer med card I got in Sept of 2013, but I will this week.
