I got my cdl back in June of 2013 but never really used it. I got my first medical card in March of 2013. The only time I really used, or tried to use my cdl was when I went to an orientation for a company in September of 2013 but I failed their road test ( bumped a curb with trailer ), while at that orientation they gave me another physical at a offsite clinic and they gave me a updated/new medical card, although they never gave me any longform with it.
Now today I got a letter in the mail from the DMV saying I need to submit updated medical card info or my cdl will be revoked by 5/5/2015. The notice they sent me says that my medical card expired last month, March. Which means they must have never gotten that I received another medical card in September of 2013 since they only have info of the one I got in March of 2013.
I'm wondering would I be able to submit my " newer " medical card that expires in September of 2015 since it's technically not expired? Or would the DMV require me to take a brand new physical? I plan on using my cdl within a year or so which is why I don't want to loose my cdl status.

Unless you can produce and submit a copy of the long form from the second physical, you are S O L.

You should submit the one you have that expires in September otherwise you get busted back to a regular license and will have to retest to get your CDL back.By September you should get a new Physical even if you have to pay out of pocket or you'll lose your CDL.The doctor or company does NOT submit them for you.That has to be done at your local DMV

Yeah I thought about that but the notice doesn't say anything about the longform portion. It says " a valid, unexpired Medical Examiner's Certificate/DOT Physical Card, "
So I was taking that as just the card, or is the longform portion also implied? -
Hmm ok. I'll have to stop by my local DMV and ask them if I need the longform portion or not. Wouldn't be a big deal but like I said I don't plan on using my cdl for about a year or so. So I don't really want to go now and drop about $100 for a med card that will do nothing for me for a year. But if I have to I will.

If you supply the certificate you have on hand. They may presume you have the long form and not ask for it. I was able to submit mine online. Just scanned it and sent via email to the DOT.
TO add I was just inspected level 3 and the Inspector asked for my medical card and said we,drivers, should carry our medical card for at least another 2 years. -
If you don't need to use your CDL isn't there a check box on the form that requires no medical card and restricts you somehow until you get a medical card and can simply take your medical card in and sign a new form and check a different box?

Don't know where you're located but here in PA I was never asked for my long form just a clear visible medical card.
