DAC reports NOT available by phoning in request

calling the number mentioned in the below post leads to an automated menu that directs the driver back to the website, [LINK POSTED BY MEMBER] Only Members Can View This Truck Forum Link. .
the recording tells the caller to go to the consumer tab at the bottom of the page and then fill out the on-line form.
***problem - there IS NO ON-LINE FORM***
you are instructed to download a pdf request form, print it out, fill it out
and manually send it via snail mail.
it will then be 15 days (3 weeks) before you get your copy.
So figure a full month from going on-line to getting the report.

There are posts below with the info that is requested in the pdf form as well as the address to mail it to if you don't have access to a printer.

edit for extra info :
The pdf form allows you to select other reports however, so I would suggest using it.
I have asked for my DAC report, my DMV record and results of drug screens (credit and criminal records checks are also available) to cover all bases.

Requesting DAC report

I don't know about sending anyone a copy of my SS Card
Ant other way to get it and how long does information stay on it

you do not have to send a copy of either your dl or your ss card
but they do need those numbers to correctly identify you.

You can get to the USIS (formally DAC) for both the request and appeal forms by going to their web site
You don't have to fax any information like the link above you will have to wait a couple of weeks for all the
paper work, I found this link a lot more helpful and I didn't have to send any info out or jump through any hoops.

Click on the above link, fill out the form (it doesn't cost any money), sign-in and that's it. You can also use the above link if you need to appeal something on your DAC. You can call the toll-free number but all they will give you is the address for the web site. Gook Luck hope this helps.
If you don't have a printer at the time go to adobe and get the FREE reader for pdf files and save it in the file. You can save both forms this way to print up later.

Check with Unemployment to see if they will pay for your training. I live in CA unemployment paid for my training, unemployment may have a program (depending if this is a new occupation for you) that will pay for the program with NO out of pocket money. Like the other person said, "Go with a school" not a name carrier (ex: Any company that is well known)
You will have to sign a contract with them stating that IF you quit them you will have to pay them thus takes away if you are not happy with the company you will have to stay or else pay. Good Luck in you venture.

Can you clarify? You've asked for your DAC report, DMV, drug screens, credit and criminal records check.

Does the DAC report cover all of these? Or are you contacting different organizations for these others you mention?

I have never driven before will the DAC report have anything on me? Can I get info from USIS too? Would it be a good idea?

I went to the website , downloaded the DAC form & sent it in by mail. Forgot which day I sent it in & called them up about tracking it. They had all the info right there & said I should recieve it on the 30th. Thats tomorrow. This will very interesting to actually see what my official public record says. I am learning you can't hide anything. Technology has gotten increasingly invasive & very up to date on all aspects of our personal lives. All the DAC report costs me was the price of the postage stamp.

Do you know if you have to use there form or can you just right them a letter with all your info on it? i went to the DAC site also. How do you find the form?

Disregard this question I sent for a form today.
