Maybe a pinhole leak in a cylinder liner due to liner cavitation....this would mean time for a rebuild

Was it milky in the valve cover? Is there any condensation on the fill tube or dipstick? If not replace the radiator cap and place a gallon jug at the end of the overflow tube and secure it. Then test run it. How much water is it losing?

I would drain the fuel filter and look for coolant, the injector sits in a copper sleeve, and if you use DCA4 they corrode away quickly.
If you go to the Diesel Garage site they have a section called Handy links and there is a writeup on how to do a proper and complete bottle test.
As suggested the Rad Cap is very important, and if it is leaking into the crankcase the oil fill plug will have a foam on it.

What kind of head work did you do? Just gaskets? Did you do all three? Did you replace any heads?

Check the water lines going to as well as the cab & sleeper heater core for leaks
