Both are a very real possibility , you might check delta-p pressure as sometimes these things can get clogged up with all the burnt stuff we have to send back thru our engines these days.
( i should add that i have completed repairs on some of these engines with no parts involved by removing the delta sensor and tubes and cleaning the soot out )
2 things i think you might try are putting the 60 in egr test range by bringing the engine to temp. and then flooring the throttle 3 times.
( i am thinking that you do not have access to Detroit software , ) so after putting the engine in test range you should hear your engine sound kind of strange and the temp. start to climb in the egr cross tube at the front top of your engine , that is going to tell you that the valve is working. DO NOT TOUCH THE CROSS OVER TO THE MIXER!! please use a temp. gun. The egr test will stop on its own or you can stop it at any time by throttling it again. let me know what happens.
the second the thing you could try comes from a 35 year driver in my fleet that currently consists of of 21 units of 20 pete 587 units with cummins ISX 2250 and one lone 2006 freight shaker with a 60. egr
That driver will not report a cough problem to his dispatcher as he is scared of sale of unit and moving into a new one that is in the shop more than out. ( the shops i talk to are very full of new technology engines ) This driver has learned to throttle slowly to let the ecu get its stuff together and make its decisions on its own. good luck , gray-c

i guess thats how emmissions control works? i have to work on this stuff daily and a lot at night. a lot of times i sub these trucks just to meet deadline, i think if i retire in 5 years as planned i will have made a lot of it since 2007. see-ya rogue -

do not take this as the truth, but i think engine temp is recorded from oil=temp as on early DD model,s
can not remember sensor location
as i started on 180 cummins and blossemed into the new isx engines that drive me insane with aftertreatment codes ,
if you cut me i still bleed detroit-green. kind of silver now and packed full of new issues now in the DD 13 , 15 engines
but i guess we have to have to keep up with the times

I have a 07 I had changed the v pod and actuater and It did that till I had it adjusted throw freight liner and now it is fine in that aspect

My 2004 Columbia, 14 ltr 60 series, is doing the same thing. Did your problem show up as a code when the unit was attached to the diagnostic computer. My truck isnt popping any codes at all.

No codes just black smoke,rough idle etc....
take flat screwdriver while engine is running and try lifting the actuator arm you will see the difference in engine idle -
A friend of mine has an 05' that was having similar issues that lead to an EGR valve replacement. But when it happened again shortly after that, a competent mechanic looked a little further and......
As it was relayed to me, there is a plunger type thing in the EGR valve itself that was getting carboned up/ gummed up. He cleaned it and apparently this cleared up all the symptoms.
