Replace your crankcase filter. Located at the left front upper corner of the engine. It is inside the plastic box that has about 10 small bolts holding it together.
-
Seconding the crank case filter. It's a regular service item on the ISX that often gets ignored until it starts throwing codes.

How often should this be done, and why is this not part of a regular PM
-
Your local Cummins dealer can let you know the exact interval, but I know the interval is much longer than a PM interval.

Interval is 125,000 miles or one year as far as I can tell.

I had to replace mine 4 times over 3 months last winter. First time the truck was 3 weeks old. Each time didn't last as long as the one before. After the last one, the code came back 8 miles down the road. Turned out the engine was defective. This was on a brand new Freightliner Coronado. The dealer told me that they would replace the engine under warranty, but the replacement engine would only have 1 year warranty. I gave back the truck and had to fight them to get my trade's value back.
