What is an "overhead?"

If you mean overhead as in 'run an overhead' it refers to a bit of routine engine work: setting valve lash to the proper tolerances.
And I'm no mechanic, so my terminology may not be right, but if Joe is lurking around he can explain it better. -
You have it right. it's the equal of setting the valves in a car's engine. You adjust all the various operating mechanism in the head, and it should be done about once a year for best performance and economy.

I worked for a company years ago and personally put 490k on one truck since new, it had a 3406 cat in it. They took it down to McAllister's and had a overhead done on it at 400K, after telling the fella the miles on it and it had never had an overhead done on it since new, he shook his head and said I'll feel a big difference when i get it back, i never noticed a difference. On the truck i have now, i prefer to maintain it better than the way they did things. I talked to Kenworth in Indy and they said for a C-15 cat it'll run around $500 that included, running the valves, the jakes, and resetting fuel pressure. Seems like a lot of money for no more than they're doing, hard to lay out that kind of money for something you may not notice, but it shouldn't be neglected.

The CAT dealer is charging you a flat rate price. It usually takes one to to hours depending on applicatioon and if it has ACERT stuff on top. I used to do them in 45 minutes with no problem. I am not sure their comment about setting fuel pressure. it is a grear driven pump and has one setting "max'. Maybe they are charging you to hook up a gauge and checking it.

yep should only take 1 to 2 hours to set valves and injectors and jakes, but they (cummins cat and the like) like to charge lots for it??? dont know how they get away with ripping so many people off. I do my Cummins M11 in about 3 or 4 beers (1.5 to 2 hours)
