Just another thought. Maybe you have a bad thermostat?
Maybe its not opening all the way?
    Maybe its stuck partially open?
Does the coolent temperature fluctaute allot or run a little warmer than usual?
What temp is your truck running at?

What is the lifespan of a 3406E rated at 355hp with 680,000 miles on it?

Mine has 1,751,000 miles on it. I rebuilt it at 1,613,000... had one liner and rods/mains at done at 1,310,000.

It also had some work done at 600,000 but that was before I owned it.

I have seen CAT "E" models run upto 980,000 miles and the only reason it didn't make a million is the crankshaft broke. HP has nothing to do with longevity. maintenance both of lubrication and cooling system are important. if you changed or had the lowerend done around 500K then you should be OK.

The comment about changing timing one tooth is definitely not a good idea. the timing sensor will never calibrate.

The comment about the injector rocker damage done after head replacement is definitely a shop redo issue. The rocker pads can't turn sideways they are either not adjusted correctly and the pad turns sideways of the pad can break. Those injector tubes can be replaced without cylinder head removal. It was probably the o-rings on the tubes that were leaking. I never saw an injector tube crack on an "e" model in all of the repairs I made in ten years.

I have a 96 volvo with a 3406e. I started the engine and everything was fine then i see oil running under the truck so i turn it off and inspect the problem. I found an o ring blown out from oil return line on turbo. I replaced o ring and refillef oil now engine won't crank. It will turn when i bump the starter so i know it's not locked up. Any ideas?

How much oil did you lose? All ten gallons?

If you lost all of it, you could have major damage even though it rotates freely.

No not all 10 gallons, just 6. I put a battery charger on it and when not on a compression stroke motor spins but on the comprssion stroke only 1 click at a time.
