Ok, so if you need both, and the op wanted to get his own shop licensed for inspections you need to follow the requirements for both Federal and the State. I guess it depends which State you live in, they aren't all the same but seem to be in the same ball park for minimum requirements for your shop.

The state inspection is required to purchase plates (tags) so it must be done when licensing the vehicle. The federal annual (DOT) can be done anytime on the calender and doesn't have to coincide with the date of the state inspection, but should be kept current. The federal annual (DOT) inspection doesn't require a shop (entity), building or special licensing to be performed. The state inspection requires a shop (business entity) licensed by the state as an Official Vehicle Inspection Station with state licensed inspectors to perform the inspections inside the shop with minimum required tooling. These inspections cannot be performed off the premises like a federal annual (DOT).

By virtue of wrenching my own truck for 17 years I do believe I am hereby qualified to perform my own inspections.
