$500 Overhead service @ Volvo OKC

    I've had this problem with EVERY truck I've ever driven. Has your drive shaft ever been replaced? It may need to be trued/balanced. It could be a bent/damaged wheel. The company I drove for last did not have one truck that didn't vibrate it's !@# off. Peterbilt was the worst by far. Has the yoke ever been removed? It may not have been reinstalled at a 90 degree angle to its mate. It really does sound like a universal joint.

    I also have had this problem with the last two trucks i've driven. Both co. trucks had everything checked ride height,carrer baring,drive line ect. The one thing they would'nt do though is replace the shocks even after i told them about the time thats what stoped the vibration in a truck i was driving a number of years ago. That was a o/o's though and i just got lucky that he took care of his equipment. i always found it funny how much companies complain about costs but won't replace a set of shocks. not to this is this is the cause of your vibration but its worth a try. Its not that exspensive. Average shocks are only good for bout 200k. (and yes your driveline can be outa time)
    Worn shocks can cost you lots of $$
