I know what the code is: manifold boost pressure/unknown root cause.
2009 vn780 D13 485 Ishift
This is what has been done to this point:
turbo and actuator
air to air pipe turbo side and boot intake side
CAC
exhaust manifold
both bellows pipes between turbo and dpf
resealed intake manifold
engine harness
both boost sensors
egr diff pressure sensor and cleaned the venturi pipe
reprogrammed engine ecu
7th injector and w/forced regen (which has no problems in passive mode)
overhead has been run a month ago with injector#3 and 6 cups
no intake or exhaust leaks at this point visible carbon deposits, bubbles, or audible.
I receive no check light when the fault occurs even though the manual says i should. The problem occurs while pulling grades loaded and empty. Oil temp will hit 230* and coolant 215* at which point i will start loosing boost pressure and truck will derate. Once oil and water temps come back to normal operating ranges the truck returns to normal operation with no problems.
anybody have any ideas what would be causing this?

FMI -11 low boost pressure. Usually wet air filter
maxya1986 Thanks this. -
air filter makes no difference.

try to replace the EGR; i had similar problems; if boost presure sensors didn't fix it;
bigdrip Thanks this. -
I have 1,074,000 miles on my "08,Volvo 780,D13; still running ok. -

You ended up with a good truck compared to my lemonPablo-UA Thanks this. -
Replaced egr finally and no love. Problem still persists going to put the truck on chassis Dino and get sensor values. I will post the final solution if I ever find one :/
