I have a 2005 volvo VED 12 and the clucth fan runs all the time. I am getting a fault code Mid 128 SID 33 Fmi 4,5 any help?

Check airline going to fan to make sure its pushed all the way in. Next check solenoid on air block under the truck. Last the fan may be bad.

Mine does not have air but in you unplug the wire connection it still runs but i don't know if it's a normally closed or normally open system?

I would say its normally closed system.. only with power on it turns off the fan..
never had problem like this so can really tell but found something similar in this post:

I just had to replace the fan hub (yesterday) on my 06 d12.... $1.200
on a d12 air to the fan hub= fan off, no air to the fan hub = fan on
1st check and see if you are getting air to the hub.... unplug the green air line going to the fan hub.... start truck, truck temp UNDER 175 with a/c off wait 30+ sec and see if you are getting air out of the green air line....yes/no...
if yes the fan hub is bad....
if no it's in the switch....
check switch... with the truck running pull the green air line, is air coming out of the switch..... yes/no
if yes then the green line going to the fan hub is bad (replace)
if no unplug the wire to the switch and check to see if it is getting power to the switch.... yes/no
if yes bad switch (replace)
if no then you will need to back at the next plug up wire until you find it
are you losing air (may or may not hear it) with the key turned on but without starting the truck 30+psi in 3min or less.... if yes it's a sure sign you have a bad fan hubcurtislo Thanks this. -
more then likely it the solenoid with two 1/4 air lines and two wires that plug into it. you will find it under the clutch fluid reservoir also under steering column next to firewall cheap fix
