Have you taken it to Volvo? I had a friend who had that problem and hes been told it had something to do with the Regen System. Havent talked to him since so I don't know if he got it fixed or not?

Yes i paid 1500$ at Volvo for nothing. My truck is 1998, there is no Regen System.
Truck running good without loaded trailer, pb appears only when the trailer is loaded. Turbo work well, there is no more "pedal" when reach on 6th speed, fuel shut off.

Might change the accelerator sensor under the Accelerator and see if that works? This suggestion would be a long shot but your exaust may be collapsing? Ive only seen that happen a couple of times but it has happened. Id try changing the accelerator sensor first. If that don't work and no one else comes up with a solution, you might try changing your muffler and the pipe between it and the exaust manifold? Ive only seen that happen on like trucks from the 60's and 70's but I guess it could happen on a 98? Volvo may have been on the right track by changing the exaust manifold but they just didn't go quite far enough?
