Attention Western Express drivers from CA!

    Just passing along some info that might help you out......

    The nurse in Nashville giving out the DOT cards is using the OLD FORMS! (They say "2012" on the bottom left of the page)....... I tell you this because the The DMV ISN'T RECOGNIZING THEM.

    If i were you, id politely inform them that they need the new 2014 form or your DOT CARD will be no good......... (dont ask me how i know lol)

    Im not upset at them or anything, things like that i guess should be expected there.... Just watch out (i didnt mention that dmv had no knowledge of this "new card" i got from western lol)

    I just got mine from my doctor in feb, The medical card doesnt have a date and the physical form has 2008 on it. I'm starting at Western Express Monday. Could you be more specific about where and what the date is on?

    look on the bottom of the long form (not your card), in the lower left corner should have a year on it...... if it says anything other than 2014 it needs to be done over...... Sorry I said dot card: its the long form thats outdated- which inturn makes the dot card invalid

    You are correct the new forms say top right corner: 501 , lower left say: DL51(REV1/2014)
    Its true CA DMV will not Honor them if its not their own Medical long forms,they told me NO out of State medical forms...Your CDL will be useless here with out 501 long form.It happen to me ,back in Jan 2014 had to take medical again pay $90.00 then apply for same job,by then company had let me go...couldn't drive out of CA for a month went with another company....Take Care out there
