The birth certificate that I have been using has somehow come up missing. I was digging through my stuff to get ready to take to truck driving school coming up in 4 days. The only thing I have now is the birth certificate that my mother has with my last name as her maiden name. I was adopted and last name was changed. She does however have the court documents stating that my name has legally been changed.

Will this fly with DOT? I don't want to drive 6 hours to training to be turned around, even though I have all legal documents.

    Please help.

It's been awhile... But I don't remember having to show my birth certificate.

Were you born locally? Go to the court house & get a certified copy.

yeah i was but I called today and the only copy that they have is the one with my last name as my mother's maiden name same as the original. To replace the one with my current last name I have to go through the state, and that takes approx. 30 business days.

I mean I think I should be fine...it's signed and dated 3 days after I was born, It is technically my real birth certificate....and I have the signed and stamped court papers with a case number of a legal name change. I just hope they accept it.

In my belief.... Court documents with a judges signature is the trump card that can & will override everything else....

    sicksfeet Thanks this.

yeah i guess that makes sense. I'm just freaking out because of the fear of being sent home. The last thing I need is for this to fall through or be postponed because of a technicality. I need my ducks in a row on this and now i'll be uneasy until I'm face to face and get an answer. Probably be pregnant with an ulcer by then.

I think it depends on the state, its regulated by the fed policy...but each state has their own version of the process. I know here in TN, they only accept a birth certificate with all the correct names and it "must" have the embosed state seal or they will not even look at it. We had someone in our class that had to rush around and spend a fortune in Fed-Ex shipments. Most states can get it for you very quickly if you call the proper goverment offices. I remember this guy telling the class it cost him like $250 to get the matter straightened out.

I'm STILL having trouble getting mine from NYC - and I applied for it back on July 15th. Thankfully, I was able to make it thru CDL training with my military ID and a copy of my DD-214 along with the receipt from Vitalchek.

I'm gonna be calling the NYCHD again today to talk to someone higher up in the chain - since the three people I've already talked to seemed more concerned on whether my NAME was on my mailbox rather than if they'd printed the right mailing address - or even mailed it at all,

Today marks the 30th business day since I applied, and my local post office says there has been more thqan enough time for it to have arrived AND been sent back - assuming they told the truth about it being mailed out on July 24th.

Do all that you can to secure the document before going for training - directly if possible rather than thru a Clearinghouse such as Vitalchek.

If you are near the hospital where you are born, they can print you out a birth certificate right then and there. I got 2 copies in less than 10 minutes.

    I don't think copies suffice.
