The only record the hospital I was born has is the same birth certificate I am holding, with my last name as my mother's maiden name. i would get a copy of what I have. To get a birth certificate with my current last name I would have to go to through the state registrar. But I did contact my recruiter and he said that as long as I have the court change of name with the proper signatures that I would be fine. And I do have those.
However, he is a recruiter.

im thinking its the legal one you were born with and its all stamped and dated so why add fire to the fuel so to speak?

If you show the birth cert. that you have, along with the court adoption records and or name change you should be fine. Actually, all they are looking for is you're right to work/citizenship.
Funny thing about this country is its harder to get a job if you are a citizen, then if you aren't!sicksfeet Thanks this. -

Do you have a passport? If so, it will work as proper documentation. Make sure that the birth certificate is the legal one, with the embossed seal.

nah i don't have a passport yet, haven't needed one. But i do have an embossed seal on both my birth certificate and the court documents on my name being legally changed.

I'm still waiting for the one that the NYCHD "claims" was mailed out on 24 July to arrive. I've talked to four different people, but for some strange reason they were concerned about whether my name was on the mailbox outside. I explained that I did not live in Petticoat Junction, and names on mailboxes are not common in my neighborhood - just the house number.
I've got the name and number of the Supervisor now and will be giving her a call in the am - since more than enough time has passed for it to be misrouted and returned to NY.
I could have driven up there and back several times by now with all this waiting. -
