Diabetic Taking Insulin-what paperwork do I need for DOT Physical?

    I believe I have all the paperwork I need to go and get a physical

    -Endocrinologist Visit and Special Form filled out saying I am safe to drive
    -Letter from my Optometrist--No Diabetic Retinopathy

    Is there anything else I need? From what I understand, that's it.
    I take that paperwork to my DOT Physical and they do a DOT physical like normal
    they have the paperwork in hand from both my doctors-- and if the physical like it normally does, I am good to drive. But, I am only cleared for 3 months. And then I go back in 3 months and they have to take a look at my blood sugar readings for the last 3 months. If everything is good then, they will give me a Health Card for longer.

    Sound about right?

    Might want to get an A1C, that's likely what they are ordering anyway with the 3 month card.
    I would call the clinic you are going to go to though. See what their policies are as it's ultimately up to the doctor issuing the card.

    Maybe I wasn't specific enough in my question. I want to know if my Testing DOT Physical Doctor needs anything MORE than what I have. (A1C, Endo Form Below and Optometrist Diabetic Neuropathy)

    I was hoping someone knew from recent experience.
    Because the form that I show below does not state ALL the paperwork I would need for the physical. Finding that seems like finding a needle in a haystack.

    and I know I can drive based on the following in the paperwork that was sent to my Endo
    -The most Relevant Part

    Note: The individual is not physically qualified to operate a commercial motor vehicle for up to the maximum 12-month period until he/she provides a treating clinician with at least the preceding 3 months of electronic blood glucose self monitoring records while being treated with insulin. At the certified medical examiner’s discretion, the individual who does not possess at least the preceding 3 months of electronic blood glucose self-monitoring records while being treated with insulin may qualify to operate a commercial motor vehicle for up to but not more than 3 months. 4. How many times per day is the individual testing his/her blood glucose? ____________________________________ 5. Is the individual compliant with blood glucose self-monitoring based on his/her specific treatment plan? _____Yes _____No Comments (if necessary): ____

    That is straight from the PDF Form on the FMCSA website and I had to read it about 20 times to get the full meaning.

    It does seem like you're right
    even with the paperwork from my ENDO and even though my ENDO knows an infinite amount more about diabetes than my DOT Physical Doctor, the DOT Doctor could still refuse 'on their discretion.'

    If my endo was doing my DOT Physical, which they do, I know that wouldn't be a problem. But they are a hour and half away, thats the kicker.
