with regards to what you said.. the name matching the tax return. it is your business being paid from the broker and you will be filing taxes on behalf of your business based on the income reported on the w9 form.. so all that means to me with regards to line 1 is make sure you are reporting on Line 1 who is being paid so you can file the taxes for the income that is being reported on the w9 form. if that makes any senseGod prefers Diesels and gekko1323 Thank this. -
So basically, because you are a SINGLE-MEMBER LLC, the answer is that you MUST put your individual name in line 1, and your SSN in Part 3.
The IRS DOES NOT recognize LLCs as tax-paying entities. LLCs are only LEGAL entities. SO any earnings must be paid by the single member. If you have more than one member then this doesn't apply. Incidentally, EIN is not even required for your business, unless you are paying people.

I have a single member LLC for a non-trucking related business. You can certainly use your SSN, but you can also get and use a personal EIN even if you don't have employee's. I use the EIN assigned to my individual taxpayer account for filling out W9 forms so I'm not giving out my SSN. I've also always put my LLC name on the form and have never had an issue. The LLC also has an EIN, but I think that was only ever used for the bank account.
