And it's not public. I've made every effort to keep myself anonymous.

I wish people would actually take the time to look up the FMCSA regulations before they render a judgment about the FMCSA and controlled substances. HAVING A NOTE FROM YOUR PHYSICIAN IS NOT GOOD ENOUGH unless of course your Physician is a FMCSA certified Medical examiner. You get your Physician to write the prescription and a note saying in their judgement you are safe using said substance. You take said prescription and note to a FMCSA certified Medical Examiner who if approves it will place a note in the physical long form stating you are medically OK to use said substance. You DO NOT need a letter from prescribing physician because this should be part of the long form if the ME approved your use. Once approved you keep your pharmacy labels on all medications. Jeez I will never understand why someone wants to always over complicate something that is PLAINLY on the FMCSA website.

This isn't something that already happened. The only part I didn't have was the note from the prescribing doc.

I want to add something else before someone gets overcome with a spell of verbosity and makes a fool of themselves. If you get stopped and your meds searched and a DEA controlled substance is found. YOU WILL be asked for your Physical long form and it had better have in it where your use of said substance is approved by the ME. If not your are subject to be placed OOS.

Made sure the vial is in the box with my name and rx info. Doc just wrote a note that it doesn't affect driving ability. That was the last hung I needed. I was concerned about the needles but I just put all of it together in a ziplock with the note. I figure it's best to have my long form at all times. Thanks for your help!!! I'm set to go on Sunday!!!!!!

There is no such thing as an "approved" DOT drug list. You are correct, though, that testosterone will not DQ you from operating a CMV.
