Many are O/O that buy retired fleet trucks and run them out ragged. So i guess it's a mix of both.
But yeah, the O/O are generally the only ones to keep old trucks around that long.
Pampered... I really don't think I'd agree on that across the board. Some OOs are horrible at maintenance...especially the cheap freight hauliers that run a unit until it falls apart... Bungee corded fairings and stuff come to mind.
Even UPS dumps their trucks once they hit 700k or before. They run their equipment longer than most fleets if it's been a good unit.

First try some Howes fuel cleaner. It may clear it up, but it's more likely you'll need to replace the FQV. It's about $750 for the part.
