Do you have to send in the MCSA 5875 form with medical certificate?

    I'm in Illinois with a CLP and just got my medical card. Do I only send in the certificate? I was also given copies (5 pages) of the exam report MCSA 5875.

    Also my CLP is NA INTRAstate only. When I send in the paperwork do I send a note to put me as NI INTERstate? Or do they do that automatically?

    Last time I got my medical card updated here in Montana (2018) the medical examiner sent it to the state. The state also just sent me a notice 2 weeks ago that I need to renew in January. I think states and examiners are different. When I lived in Iowa I had to send proof in to the state Dot.
    Not sure about the intrastate note. I always said interstate even if I was intrastate, that info should be on your license.

    The FMCSA has a lot of information on their website. I do not know how old this information is. However, there is contact information provided. I highly recommend going this route if you have any doubts.

    Emailed the certificate only, looks like no need to send the 5875 forms, but was told if I want INTERstate I have to go to a CDL facility to change to INTERstate with the certificate.

    I was told I could only self certify as INTRAstate when getting my CLP.
