If you are leased to a company, then yes you must have "operated by" above the company name. City and State are not required. FMCSR 390.21(b)(3)

Last time I was leased onto a company. Owner and Operated above my name and city on the door.
On tool box doors was Lease To: ABC co. Followed by required info.
I allways made thier name smaller but legal. Just to make a point on who makes the decisions. -
