Louisiana Trucking Company Employee? Travel mug for a survey?

Hello! My name is Nila and I am a research representative from Louisiana State University.
We are offering LSU Agcenter themed travel mugs for Trucking companies representatives willing to fill out a quick 15 mins survey about health promotion initiatives.

If you are older than 18 years old, working for a Louisiana trucking company (except truck drivers) and can make decisions (large or small) about offering or not offering health promotion programs or would help implement a program at any capacity (e.g., making decisions, training, delivering the program, recruiting employees, reports, presentations, funding, paperwork, networking, etc.)?
